More than 34 million Americans have diabetes, which is nearly 11 percent of the U.S. population, and 48 percent of U.S. adults have some form of cardiovascular disease. To prevent, manage, and minimize these chronic conditions, we must turn to nutrition.

Healthy diets are scientifically proven to help reduce the risk of chronic conditions like heart disease and diabetes, but healthy eating isn’t always easy or possible, especially for low-income individuals and families.

While the research on food as medicine is clear, the process for getting nutritious foods to low-income households is still being tested.

Attend our webinar to learn about the evolution of food as medicine and how to implement a successful food as medicine program.

Topics Include:

  • Step 1: Demystifying the myths around food
  • Step 2: Tapping into predictive analytics to identify food insecure members
  • Step 3: Creating innovative and personalized food as medicine programs
  • Step 4: Building the infrastructure to scale your program
